This template includes (emboldening in original):

This article may have been created or edited in return for undisclosed payments... It may require cleanup...

and we have been told in previous talk page discussions that (for example) "It doesn't say that there is a problem, but it makes it clear that there is potential for one".

Generally we never know with 100 % certainty which is why it says "may have", but it's typically because accounts have been found to be socks via an WP:SPI, or because people have found adverts by companies looking to create articles on freelancer sites, or because a username is indicative of someone being an employee of the company, or the article history / user history contains various hallmarks that are indicative of paid editing.
